The reality is that all of us have a hard time completing our jobs successfully at one point or another. We're only human, after all. Fail is always lurking right around the corner. You just hope that your moment of forgetfulness and the failure that results doesn't end up looking like a whole branch of a bank being left open for any criminal to cruise in and take a crack at cracking open that safe. Of course this is the internet, and people are always searching for new ways to drive their clout up. Maybe this is staged.
